Media Consent Form


As the parent or guardian of a participant in the Real Madrid Foundation Football Program

Malaysia, I hereby agree and give consent to the following: 


 1. Your data may be processed by the Fundación Real Madrid (FRM) with Tax ID (CIF) G81828493 and registered address at Av. Concha Espina 1, 28036 de Madrid, Spain; and Winner Takes All Sdn Bhd (WTA) with Company registration number 1365305-M and registered address at Suite 6.03 Grand Millennium Hotel Kl (OƯice Block), No. 160 Jalan Bukit Bintang, 55100 Kuala Lumpur, Malaysia, and the single purpose and legitimation for the processing of the personal data is the management of participation in the activity. Processing of the data may entail international data transfers. The data concerned will be stored for as long as the consent has been granted, or, where applicable, the legal period established for each of the legal obligations derived from the processing for which the consent has been granted. Likewise, you will be able to exercise their rights to access, rectification, opposition, limitation, erasure, portability and the right not to be subject to automated decision-making by contacting the Fundación Real Madrid via email at: cumplimiento.fundacion@corp.realmadrid.com or in writing at their registered address with copies sent to rmfmalaysia@drmgroup.com.my


2. The Fundación Real Madrid and/or WTA will be able to take and use image and/or voice, via recordings, interviews or photographs, in order to promote the Fundación and/or WTA’s activities, as well as for training purposes in the form of presentations and/or promotional videos and any other type of audiovisual output, its reproduction and subsequent publication across different media and communication channels. This authorization is subject to no monetary compensation and has no geographical or time limitations, all with the sole exception and limitation of infringing on the right to freedom from injury to honor.


3. The transfer of data to the Fundación Real Madrid, Real Madrid C.F (CIF G28034718 and registered address at Av. Concha Espina 1, 28036, Madrid) and WTA, for the management of your participation in a range of events and activities, the management of the Madridista card, carrying out opinion polls, as well as for the sending of commercial communications, including via electronic means.


4. The transfer of image and/or voice data to organizations directly or indirectly related to the Fundación Real Madrid and/or WTA in order to promote the activity itself or any other Fundación Real Madrid and/or WTA operations.


5. The Fundación Real Madrid and WTA commits to eliminating all data including the images and/or voice of the students when they cease to be useful and/or when the consent for the purposes they were transferred has expired.

Health Declaration & Consent


As the parent or guardian of a participant in the football training program, I hereby declare and

agree to the following:

  1. 1. My child / ward is in good physical and mental health to participate in football training
  2. My child / ward has no known medical conditions, allergies, or recent health concerns that would prevent or limit participation in physical activities.
  3. My child / ward is not currently taking any medication that may aƯect their ability to safelyparticipate in football activities.
  4. 4. I authorize the training staƯ to arrange medical treatment for my child / ward in the event
  5. 5. I understand the risks involved in football training and agree to allow my child / ward to
  6. I agree not to send my child / ward to the training sessions if they exhibit symptoms of any communicable disease, including but not limited to fever, coughing, or other signs of illness.
  7. I will promptly notify the organizers if my child / ward becomes ill or is diagnosed with any communicable disease after attending any training session.
  8. I acknowledge that the organizers have the right to deny my child / ward access to the training sessions if they have a reasonable belief that my child / ward poses a health / safety risk to other participants or staff.
  9. I understand that the coaches, staff, agents, and subcontractors of Winner Takes All Sdn Bhd are not medically trained except for the ISKL Community CPR Course.
  10. As the parent or guardian, have read, understood, and agree to the above Health Declaration & Consent. I confirm that I am responsible for the child / ward’s participation and that the information provided is accurate and complete to the best of my knowledge.

 Real Madrid Foundation (RMF) Football Program Malaysia

Terms and Conditions

The RMF Football Program is available for participants who agree with all the terms and conditions

outlined in this document. If there are any points you do not agree with, we kindly suggest reconsidering

whether this program is the right fit for you.

General Terms

1. All passes are personal, non-transferable, and non-refundable.

2. All unused sessions or no-shows will be automatically and strictly forfeited, and will not be

refunded, replaced or carried forward.

3. All listed fees exclude applicable taxes, which will be charged accordingly.

Eligibility

1. The Football Program is open to all boys and girls aged 5 to 17 years (including non-ISKL

students).

2. In compliance with applicable regulations, the RMF Football Program in Malaysia cannot

accept players who are 18 years or older.

3. Participants who will turn 18 during the subscription should contact the management team

for further guidance.

Training Passes

4 Training Passes are available to interested players:

1. Free Trial Pass

2. Monthly Pass - Once-a-week training (Development / Competitive)

3. Monthly Pass - Twice-a-week training (Development / Competitive)

4. 6 Months Pass - Once-a-week / Twice-a-week

1. Free Trial Pass

a. Available only to new participants.

b. Entitles the participant to one trial session within their age group.

c. Registration and pre-booking are required; walk-ins are not allowed.

d. The official RMF jersey kit is not available for purchase with this pass.

e. RMF insurance coverage does not apply. Parents/guardians are fully responsible for

any injury or damages incurred during the session.

2. Monthly Pass - Once-a-week Training (Development /

Competitive)

a. Allows one (1) training session per week on a selected Preferred Training Day (PTD).

b. Purchase of the oƯicial RMF jersey kit is mandatory (sold by management separately;

details available on the website).

c. Participants must choose one PTD and commit to attending on that day throughout

the term.

d. PTD changes are allowed for medical reasons (with a valid Medical Certificate) or

compassionate grounds, subject to management approval.

e. If a player misses a class voluntarily, no replacement session will be provided. Only

sessions cancelled by management will be replaced.

f. All missed sessions or no-shows are forfeited.

g. Payment must be made through monthly automated recurring payments using a valid

credit or debit card. Manual payments (cash or bank transfer) are not accepted.

h. Special Note for Competitive Players:

i. If a competition takes place during scheduled training days, that day will be

considered as a training session.

ii. Replacement session will not be provided for training missed due to

competition participation.

3. Monthly Pass - Twice-a-week Training (Development /

Competitive)

a. Allows two (2) training sessions per week on selected PTDs.

b. Follows all the same terms as the Monthly Pass - Once-a-Week Training, including

mandatory kit purchase and automated payment.

4. 6 Months Pass - Once-a-week / Twice-a-week

a. Paid in full upfront for a six-month period.

b. The six-month duration begins from the date of payment and runs consecutively.

c. Freezes, extensions, or transfers are not allowed.

d. Follows the same terms as the respective Monthly Passes.

e. Payment must be made via cash or bank transfer only.

f. A cancellation will incur a penalty equivalent to one month’s fee.

Competitive Players – Fees and Commitment

1. The RMF Football Program does not charge players for participation in competitions.

2. However, all relevant tournament-related expenses—such as registration fees, referee

fees, facility rental, transport, and other shared costs—will be divided equally among all

selected Competitive Team players.

3. A detailed breakdown of costs will be provided ahead of time, and payment must be made

by the given deadline to confirm participation.

4. Commitment Requirements

a) Players selected for the Competitive Team are expected to fully commit to attending

all scheduled matches.

b) Only injuries (with a valid Medical Certificate) or emergencies will be considered as

valid reasons for absence.

c) Competition players must also attend all relevant training sessions as scheduled by

the coaches.

d) Frequent absences from training or matches may result in removal from the

Competitive Team, at the discretion of the coaching staƯ and management.

Subscription Freeze (Monthly Pass Holders Only)

1. Players on a Monthly Pass may request a subscription freeze for a minimum of one (1) full

calendar month. Partial freezes (e.g. 1 or 2 weeks) are not permitted.

2. A maximum of two (2) freezes are allowed per calendar year.

3. Requests to freeze must be submitted at least two (2) weeks prior to the next scheduled

payment.

4. If your freeze request is submitted after your card has been charged, no refund or

replacement will be issued.

Cancellations

1. To cancel your subscription, you must provide at least two (2) weeks' notice before your next

scheduled payment.

2. If your cancellation notice is received after your card has been charged, no refund will be

issued.

Example:

If your next payment is scheduled for 30th April 2025 (for May training), you must notify

management by 15th April 2025 if you wish to cancel.

Preferred Training Day (PTD)

1. Upon registration, players must select a PTD from the oƯicial training calendar.

2. Players are expected to attend sessions on their selected PTD consistently to ensure

proper class allocation and planning.

3. Requests for changes to PTD are limited and subject to availability. Contact management

or your coach for approval.

4. Attending training on a day outside your assigned PTD is not allowed.

5. Missed sessions cannot be replaced by attending on other days.

NOTE: The OƯicial Training Calendar is always updated on our website www.rmfmalaysia.com.

Please check it regularly to stay up to date on the training days.

Changes to Preferred Training Day (PTD)

To request a reasonable one-oƯ change, please contact management or the coaches. Changes

will only be made if slots are available.

Bad Weather Policy

 Our training sessions may be aƯected by bad weather.

 If the lightning siren is activated, all sessions will be halted immediately. Training will

resume indoors, and players must change to approved indoor shoes.

 If it rains before training begins, sessions will continue either outdoors or indoors,

depending on the severity of the weather at the coach’s discretion.

 In case of rain during training:

o Light rain: Training continues outdoors.

o Heavy rain: Training continues outdoors or moves indoors at the coach’s

discretion.

 In cases where indoor courts are unavailable:

o If the session prior to the halt has lasted less than 35 minutes, a replacement

class will be issued, valid for 30 days.

o If the session lasted more than 35 minutes, no replacements will be provided.

IMPORTANT NOTE: Classes will only be cancelled if specifically announced by the coaches or

management. If you choose to miss a class due to the weather, when indoor facilities are available,

the absence will be counted as a no-show. No replacements will be provided.

Attire

1. All players, except Trial Pass participants, must wear the oƯicial RMF Educational Football

Program kit during training without exception.

2. The kit must be purchased separately and is not included in the training fees.

3. Kit includes: RMF Adidas jersey, RMF Adidas shorts, and RMF Adidas socks.

4. Wearing oƯicial Real Madrid FC jerseys (past or current) during training is strictly

prohibited as per our programme regulations.

Shoes

All players must bring two pairs of shoes for every session:

a. Outdoor football boots: Suitable for artificial turf (no metal studs).

b. Indoor shoes: Non-marking soles required for indoor courts during lightning alerts.

Refer to coaches for examples of appropriate footwear.

Parental Conduct Guidelines

To foster a positive and productive learning environment for all participants, we kindly ask for

your cooperation in supporting our coaches and players during training sessions. Our coaching

staƯ consists of highly qualified professionals with extensive training in player development,

physical education, and coaching methodologies.

To ensure a harmonious and focused training atmosphere, we encourage parents to:

 Respect Coaching Decisions – Trust the expertise of our coaches in matters such as team

formation, player placement, and training drills.

 Refrain from Sideline Instructions – To avoid distracting players and disrupting sessions,

please refrain from shouting instructions or criticisms from the sidelines.

 Minimize Interference – Allow the coaches to lead the session without interruption, ensuring

players remain focused on their development.

Safety Reminders:

 Supervision for Young Players – Parents of children under 12 must remain present to pick

them up after training. Players are not permitted to leave the field unattended at any time.

 ISKL School Policy – For ISKL parents, please ensure children are not left unsupervised on

school grounds between school hours and the start of training. Timely pickups are essential

for your child’s safety.

Your support in following these guidelines helps create a respectful and encouraging environment

where all players can thrive. We appreciate your understanding and cooperation!

Program Policies & Guidelines

Suspension and Expulsion

To ensure a positive and respectful environment for all participants, we reserve the right to

suspend or expel any player or parent whose conduct significantly disrupts the harmony of the

program. In such cases, no refunds will be issued.

Communications

We strive to keep parents and players informed of all training updates and important

announcements. Please ensure your contact details—particularly your ‘WhatsApp’ linked mobile

number are up to date, as this is our primary communication channel.

Liability Waiver & Assumption of Risk

By enrolling in this program, you acknowledge that football training involves inherent risks of injury.

While we take precautions to minimize these risks and maintain appropriate insurance coverage,

accidents may still occur.

 Supervision: StaƯ will exercise reasonable care during sessions, though there may be times

when players are unsupervised.

 Release of Liability: You agree to release, indemnify, and hold harmless ISKL and Winner

Takes All (WTA) Sdn Bhd, including their employees, directors, and aƯiliates, from any claims

arising from injuries or accidents, except in cases of proven gross negligence.

 Emergency Care: In case of injury, we will make every eƯort to contact you immediately. If

unreachable, you authorize us to administer first aid/CPR or arrange necessary medical

treatment.

ISKL’s Role & Facility Use

The International School of Kuala Lumpur (ISKL) provides the training facilities but is not

responsible for player supervision, conduct, or safety during sessions.

Emergency Contact & Medical Information

You must provide current emergency contacts and relevant medical details for your child. Failure

to do so may delay necessary care.

Photography & Media Release

You grant permission for your child to be photographed or recorded for promotional, educational,

or media purposes as required by WTA and/or RMF. These materials may be used across various

platforms without additional consent.

Appendix A

Please read and understand the Terms of Use of ISKL’s Facilities listed in Appendix A. Your

agreement to this is mandatory for your participation in this football program.

Disclaimer

To ensure the eƯicient operation and continual improvement of the program, Winner Takes All

Sdn Bhd reserves the right to make reasonable amendments, additions, or rearrangements to this

document as necessary, without prior notice, except for publishing the most current version on

this website. It is your responsibility to stay informed of any updates by regularly reviewing the

latest version.

APPENDIX A

TERMS OF USE OF ISKL’S FACILITIES

1. The lanyard with a security tag issued by the ISKL Security team must be worn and visible at

all times for identification purposes.

2. All baggage will be inspected by ISKL’s security personnel upon entering ISKL’s campus /

premises.

3. Parking on campus is subject to the school parking regulations.

4. Smoking / consumption or use of drugs / gambling anywhere on the ISKL’s premises, including

car park areas, is strictly prohibited.

5. Consumption of alcohol is strictly prohibited unless otherwise permitted in writing by ISKL.

6. Weapons or any items that pose health and safety issues are strictly prohibited on ISKL’s

premises.

7. Unlawful, illegal or prohibited or immoral activities/behaviours deemed oƯensive is strictly

prohibited.

8. No loitering on ISKL’s campus.

9. Student washrooms are prohibited from being used by adults.

10. There will be no tacking or nailing of any signs or other materials on walls, floors, ceiling. No

part of the building can be defaced. If applicable, rectification costs for damages will apply.

11. Rubbish/waste must be segregated by “recyclable” and “non-recyclable”.

12. All incidents, accidents, injuries, or any incident which has the potential to cause, but does

not result in injury or damage must be promptly reported in writing to an ISKL representative

within 24 hours.

13. ISKL shall not be held responsible for any damage/loss of the user’s or Participants’

belongings.

14. Players shall adhere to all instructions relayed by ISKL’s security personnel and/or the public

announcement system.

15. ISKL has emergency protocols that WTA is obliged to inform its employees and Participants.

The emergency protocols are as follows:

a. EVACUATION – If there is an EVACUATION announcement, please proceed to the

assembly point specified.

b. LOCKDOWN – If there is a LOCKDOWN announcement, please lock the door of the

space you are in, lower or close any blind, look for a ‘Safe Corner’, and turn out the

lights and computer monitors. If you are outdoors, make your way to the nearest

building and follow the instructions above;

c. LIGHTNING – The audible warning (horn) will sound and yellow flashing lights will

warn of impending lightning. Please proceed to an area under cover away from the

open fields, until the warning lights switch oƯ.

16. Parking charges (if any) will be borne by the organiser and its Participants.

17. Participants are permitted to only use equipment/parts/furniture that has been approved by

the ISKL Facilities & Maintenance Department. Any equipment or furniture that has been

moved during the Event is to be returned to their original place of storage.

18. Items may not be stored overnight without the prior written consent of the ISKL Facilities &

Maintenance Department.

19. No one is allowed to cook with open fire flames unless otherwise permitted in writing by ISKL.

20. Following the sessions, the space must be left clean, and items arranged in an organised

fashion, if applicable.

21. It will be necessary to comply with all applicable safety, health and security protocols, as

prescribed by ISKL and / or the Malaysian authorities.

22. Participants must bring their own drinking water, food, clothing, face towels. The sharing of

such personal equipment is discouraged.

23. Players and those accompanying them need to leave the activity area / school immediately

after the activity.

24. Players and accompanying party shall be confined to the allotted areas times and shall not

encroach into any other areas unless prior approval is given.

25. There must be adult supervision at all times when there are minor players at the training

sessions.

26. Except for players engaging in sport activities, the lanyard with a security tag issued by the

ISKL Security team must be worn and visible at all times for identification purposes.
